To help us better serve you, read these helpful tips to make the most of your visit.
Before Your Visit
- Insurance card and driver’s license
- Referral form if your insurance plan requires it
- All medications, vitamins and herbal supplements or a list of these items, including dosage and frequency
- X-Rays and MRIs you’ve had taken that may pertain to your problem
- Medical records from the emergency room or other physicians that may relate to your problem
- List of all of your doctors, including addresses, phone numbers and fax numbers
We treat patients from 8:30am – 5:00pm, Monday – Friday, except for holidays. We ask that you check in 15 minutes prior to your appointment. Should your arrival time be delayed, please call 386-774-2500.
Patients under the age of 18 must be accompanied by a parent or legal guardian.
If you need to cancel your appointment for any reason, please let us know as far in advance as possible. Your call to cancel/reschedule your appointment will allow another patient to receive care.
Our Call Center Specialists are available from 8:30 am to 5:00 pm (EST), Monday through Friday at 386-774-2500.
Florida Orthopaedic Associates offers same/next business day appointments across our four offices. Depending on the level of care, generally, if you call before noon, you will be offered a same-day appointment. However, if you call after 12 pm (noon), you may be offered a next-day appointment.
All same-day appointments are subject to availability of doctors across our locations and will vary based on their clinical specialty. If you prefer to see a specific doctor or prefer a certain location, you may decline the same-day appointment and schedule the next available appointment based on your preferences.
If you receive a same-day appointment, we cannot guarantee that it will be with your current or preferred physician, or at the location of your choice.
We offer a secure patient portal that gives patients an online method of viewing their medical records and date of their next appointment.
After making your appointment, please look for an email from PatientPortal@Exscribe.com in your inbox for access instructions to our patient portal. Once logged in, new patients are encouraged to complete their medical history forms up to 3 days prior to their initial office visit and submit them to us via the portal.
Please note the following guidelines regarding the use of the portal messaging function:
- Do not use this portal to communicate an emergency, call 911!
- Do not use this portal for after surgical care or questions, call our office.
- Your message will be read by a member of our staff.
To refill a prescription, please call our offices during regular business hours and select the prescription option #3 from the menu. We routinely check for messages throughout the day and will process your request as soon as possible. Prescription refills can take 24-48 hours to process.
To request disability forms and attorney reports, please call and ask to speak with our medical records department. Once we have the appropriate authorizations, we will fulfill your request in a timely manner. Please note, there is a fee for completing disability forms.
